We request that you pick up a Request for Records form to indicate what you will need copied. There is a charge of $0.25 per page. If you prefer Niu Valley Middle School to mail your child’s transcript we require you to bring in a legal sized stamped addressed envelope. Transcript fees are $1.00. For all requests listed above, please allow 2 working days to process. IDs are required. For any teacher/counselor/administrator recommendations, please have your child speak directly with the teacher/counselor/administrator and provide a legal sized self-addressed stamped envelope.
