Attention Parents:
Are you planning to move out of district, state, or country? Has your child been accepted to a private school for the school year 2020-2021?
To assure proper transfer of your child’s records, please fill out and return the “Intent to Withdraw” form. This form must be completed prior to generating your child’s “Certificate of Release”. HIDOE schools require a “Certificate of Release” for any new enrollment.
Please complete and submit “Intent to Withdraw” form by May 15, 2020 to Niu Valley Middle School’s registrar: robin_uyeunten@nvms.k12.hi.us.
